Data Privacy
All personal information is held securely and accessed only by the Parish Nurse and will not be shared unless consent has been given for this or there is a legal requirement to do so.
The person being referred has given consent for their information to be give to the Parish Nurse and for this information to be recorded and stored securely by the Parish Nurse.
Where, after consulation with the referred person, our Parish Nurse feels the needs of the referred person can be better met by another agency, or an agency to work with the Parish Nurse to secure necessary services, permission will be sought to pass on relevant details to secure services.
The person being referred has given consent for their information to be given to another agency where appropriate.
